LLM Clinical Notes Summariser โ FHIR Converter
Unstructured doctor notes โ structured FHIR R4 records โ 60% less admin time
๐ฉบ The Problem
Clinicians spend 50% of their time on paperwork โ not patients.
A typical hospital note looks like this:
Pt 67yo M c/o chest pain x3d, radiating to L arm. Hx HTN, DM2,
smoker 30pk-yr. BP 158/94, HR 88. ECG: ST depression V4-V6.
Trop I 0.8 โ 2.1 (rising). Started aspirin 300mg, heparin gtt.
Plan: cardiology consult, ECHO, consider cath.
This is clinically rich โ but computationally useless. It cannot be queried, aggregated, billed, or shared automatically.
The cost:
- $125B/year in physician burnout and inefficiency (USA)
- 3โ4 hours/day per clinician lost to documentation
- Diagnostic errors from missing structured history
- Interoperability failures โ hospitals cannot share records
โ The Solution
A fine-tuned medical NLP pipeline that:
- Parses raw clinical text โ any format, any specialty
- Extracts medical entities: diagnoses, medications, vitals, labs, procedures
- Normalises to standard codes: ICD-10, SNOMED-CT, RxNorm, LOINC
- Generates valid FHIR R4 resources: Condition, MedicationRequest, Observation, Procedure
- Validates the output against FHIR R4 schema
- Exports as JSON ready for any EHR (Epic, Cerner, OpenMRS)
Raw note (free text)
โ NLP Entity Extraction
Medical entities (diagnoses, meds, vitals, labs)
โ Code Normalisation
ICD-10 / SNOMED / RxNorm / LOINC codes
โ FHIR Mapping
FHIR R4 Bundle (Condition + Medication + Observation + Procedure)
โ Validation
Valid HL7 FHIR R4 JSON โ any EHR system
