Request to reset request form or provide access to SAM 3.1 model

Hi,
I was rejected from accessing the model after filling in the form, I did not intentionally fill it out incorrectly if that was the reason for the rejection.
Would it be possible to please have the request form reset or to provide access?
I'm building a free and local dataset engineering platform where I'd like to implement it but without access I can't test and use it properly.
