Entities in Call Centre data.

I'm trying to perform NER on the healthcare call center data about customer complains, 'why people are calling them' to be precise.
What are some entity_types that i can use? I've tried to modify the entity prompt eg: What {device} is causing the problem?, What describes the {issue} in the text?, What describes the {problem} in the text? etc.
How can i leverage 3-4 entities together in a single instance? Any suggestions would help.
