The face doesn't change... any idea why? It worked once at some point

Can you provide details on why a face swap might not work e.g. the source remains exactly the same?
I tried smaller images, and tried 2 images that have basically the same alignment... it still returns the source unmodified

There might be a misunderstanding here. But firstly, the source is always unchanged. Target is the image that is to be manipulated based of the source identity.
There are rare occations where, if the model consider source and target to similar, it will make no changes. Meaning that if target = source, you will get a (almost) perfect reconstruction.
But this could happen if just source and target is similar in identity.
