Output from medium sized image is flawed?

I'm running this locally and tried to use an input image that was 720x576, and the output appears washed out and with a grid-like pattern over it. This was generated with the default settings. Is there a way to fix this? Thank you.

Similar problem here - using defaults - and anything else i try!
